All information must be completed and is necessary to process your application.

Membership dues for August 1, 2011 to July 31, 2012, and are $250.00 per member.
Companies may have more than one member, but the $250.00 is a per member fee.
Dues must accompany completed form.

Each individual member will receive a discount of $100.00 of the cost for a booth when their company exhibits at annual conference. Also, member status may be used in advertising or exhibiting and will be noted in the Conference Program.
