To become a Sustaining Member, please fill out the form below and click submit, or download the application, fill it out and mail it in.

Application for Sustaining Membership

  • (i.e. field, beverage, equipment, software, etc.)
  • All information must be completed in order to process your application.
    Membership dues are for August 1, 2020, to July 31, 2021, and are $250.00 per member. Companies may have more than one member, but $250.00 is a per-member fee. Dues must accompany the completed form.

    Each individual member will receive a discount of $100.00 from the cost for one booth space when their company exhibits at the annual conference. Additional booths will be at the conference price. Also, member status may be used in advertising or exhibiting and will be noted in the Conference Program.
